A new website has been established to allow providers to update their contact information including provider name, point of contact for medical records, address, email address, phone, and fax numbers to ensure medical record requests from the CERT Documentation Contractor (CDC) are sent to the appropriate individual. The website address is http://www.certprovider.org.
Medicare providers can confirm that the CDC has the correct contact information by clicking on the Provider Address Directory link at the website location. On the Provider Address Directory page, the provider will enter their Medicare Provider Number to initiate a search for their contact information. If changes are needed to the listed information, providers can contact the CDC at 301-957-2380 or by clicking on the link to send the CDC an email with the updated contact information. If the provider is unable to locate their contact information in the CDC database, the provider is encouraged to submit the information to the CDC.
